Yes, I think it's a good idea to start a new thread and it is a good question.

most of the mirrors are run by third-parties. sometimes a mirror goes down after we produce an iso. When we update isos, we do try to remove non-working mirrors. I removed several with the 19.2 series. So we either missed one, or they went down later.

the live system tries to guess a close mirror. its not perfect, and it doesn't always select the fastest mirror either because at the time the setup is made, no internet connection exists. but it works more often than it doesn't.
